Community Midwives

Our community midwives run ante-natal clinics from SureStart and Sainsbury’s. They also operate out of the maternity unit at QEQM Hospital where antenatal classes are held. They are able to visit you in your own home before and after the birth of your baby. If you become pregnant and want to self-refer, and/or you wish to contact the Midwives,  you can do so online here.

 

Community Nurses

Our district nurses are part of the Primary health Care Team. They are highly qualified nurses who have undergone further extensive training to enable them to specialise in providing high quality care to patients in the Community. Home visits are made at the discretion of the District Nurse and are usually only made to patients who are unable to get out of the house, or have been assessed by the District Nurse as having a physical and/or medical problem which prevents them from attending the surgery.

They assess the needs of patients in the home environment and implement a high standard of nursing care according to identified needs and refer to other specialist nurses and agencies as appropriate.

Health Visitors

The Health Visitors and their team are part of the Primary Health Care Team. They are registered nurses with additional training in community-based health care, giving advice and support to children up until school age. Parenting courses/information, postnatal advice. groups, assessments of children’s development and links to other agencies are available through the health visiting team Contact 03000 134740.

Social Prescribers

Social prescribing is a key component of Universal Personalised Care. It is an approach that connects people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing.

In social prescribing, local agencies such as local charities, social care and health services refer people to a social prescribing link worker. Social prescribing link workers give people time, focusing on ‘what matters to me?’, to coproduce a simple personalised care and support plan, and support people to take control of their health and wellbeing.

Social prescribing link workers also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners.

Social prescribing is an all-age, whole population approach that works particularly well for people who:

  • have one or more long term conditions
  • who need support with low level mental health issues
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing.
